Zusammenfassung
Es wird die pränatale Diagnostik zweier Fälle von Holoprosenzephalie geschildert. Die typischen, bereits im II. Trimenon erhebbaren sonographischen Befunde waren: Mikrozephalus, fehlendes Mittelecho, Hy-potelorismus und ein typisches völlig abgeflachtes Profil ohne entsprechende Ausbildung der Nase. Beide Feten hatten eine mediane Lippen-Kiefer-Gaumen-Spalte, einmal wurde sie pränatal erkannt. In einem Fall konnte eine okzipital gelegene intrakranielle Zyste festgestellt werden. Die genetische Untersuchung der Feten durch Nabelschnurpunktion und Amniozentese ergab bei einem Fetus eine Translokationstrisomie 13 bei einer balancierten Translokation 13/14 der Mutter. Dieser Fetus hatte auch - im Gegensatz zum anderen Feten mit einem normalen Karyotyp - zusätzlich mehrere Mißbildungen im Urogenital- und Intestinaltrakt. In einem dritten Fallbericht wird ergänzend die pränatale Diagnose einer Zy-klopie bei freier Trisomie 13 beschrieben. Die pathologisch anatomischen Befunde werden geschildert und die Kriterien der pränatalen Diagnosesicherung mit jenen der in der Literatur geschilderten Fälle von Holoprosenzephalie verglichen.
Abstract
Two cases of prenatal diagnosis of holoprosencephaly are described. The characteristic sono-graphic findings in the second trimester were: microce-phaly, absence of the midline echo, hypotelorism and typically flattened profile without sufficient configura-tion of the nose. Both foetuses had midline facial clefts, one of which was diagnosed prenatally. One case demon-strated a dorsal intracranial cyst. Analysis of the ka-ryotype after foetal blood sampling and amniocentesis revealed translocation trisomy 13 in one case. The mother of this foetus had a balanced translocation 13/14. This foetus also had many extracranial abnormalities in con-trast to the other foetus, which had a normal karyotype. Additionally we report of the prenatal diagnosis of a third case with cyclopia and trisomy 13. The findings of the autopsy of the foetuses are described and the criteria for prenatal diagnosis are compared to those found in the literature.
