Abstract
In this complete survey of publications concerning therapeutic sleep deprivation (including reports of a conference held in 1985) first the practical management and especially the advantages of sleep deprivation of the second half of the night are explained. After discussing the psychological conditions and methods of therapeutic evaluation the treatment results in endogenous depression (melancholia), the effectiveness of repetition and of combination with antidepressive pharmacotherapy, the comparison with electroconvulsive therapy and the predictors are described. Sleep deprivation is indicated in nearly every therapeutic situation, including drugresistent melancholia, furthermore in severe neurotic depression and in depressive states of schizophrenic patients. Side effects are unimportant, and complications are almost absent. - In addition psychophysiological, neuroendocrinological and biochemical investigations are reported, as far as they concern either the mechanism of action of sleep deprivation or chronobiological hypotheses of depression.
Zusammenfassung
Diese vollständige Übersicht der Publikationen zum therapeutischen Schlafentzug (einschließlich der Berichte bei einer Tagung 1985) beschreibt zuerst die Durchführung und dabei insbesondere die Vorzüge des partiellen Schlafentzuges in der zweiten Nachthälfte. Nach Erörterung der psychologischen Bedingungen und der Methodik der Erfolgskontrolle werden die Behandlungsergebnisse bei endogener Depression (Melancholie), die Wiederholbarkeit und Kombination mit antidepressiver Pharmakotherapie, der Vergleich mit Elektrokrampftherapie und die Prädiktoren besprochen. Schlafentzug ist praktisch in jeder therapeutischen Situation indiziert, auch bei therapieresistenten Melancholien, des weiteren bei schweren neurotischen Depressionen und bei Depressionszuständen von Schizophrenen. Die Nebenwirkungen sind harmlos, Komplikationen kommen praktisch nicht vor. - Weiter wird über psychophysiologische, neuroendokrinologische und biochemische Untersuchungen im Hinblick auf den Wirkungsmechanismus des Schlafentzuges und auf chronobiologische Depressionshypothesen referiert. Abschließend wird wegen der Zurückhaltung gegenüber ,,Schlafentzug" die Bezeichnung ,,Wachtherapie" befürwortet.
