Zusammenfassung
Die orale Prämedikation mit Benzodiazepinen hat sich als klinische Routinemethode etabliert. Das unterschiedliche pharmakodynamische Profil der zur Verfügung stehenden Substanzen macht eine gezielte Auswahl für bestimmte Patientengruppen erforderlich. Wir berichten über einen Fall von respiratorischer Insuffizienz nach Flunitrazepam 1 mg oral bei einem Patienten mit intrakranieller Neoplasie. Unter Berücksichtigung dieser Komplikation sowie der vorliegenden Untersuchungen stellen wir zur Prämedikation neurochirurgischer Patienten die orale Gabe kurzwirksamer Benzodiazepine mit ausgeprägt anxiolytischer und schwach sedierender Komponente vom Typ des Lormetazepams zur Diskussion.
Summary
Oral premedication with benzodiazepines is well established in clinical anaesthesia. Appreciation of the specific pharmacodynamic profile of available drugs suggest individual prescription for certain groups of patients. We report a case of severe respiratory depression following oral premedication with flunitrazepam 1 mg in a patient with intracranial neoplasma. With respect to this complication and a review of the literature we suggest administration of short acting benzodiazepines with pronounced anxiolytic and moderate sedative properities (lormetazepam type) for neurosurgical patients.
