Long-term results and pregnancy rate after myomectomy

Objective:

The purpose of this study was to examine the long-term outcome and pregnancy rate after myomectomy.

MATERIALS AND Methods:

In this retrospective study all patients who underwent a myomectomy between1996 and 2004 at the University Hospital of Vienna, Dept. of Gynaecological Endocrinology, were included.

Results:

Totally 330 myomectomies were performed during the above mentioned time period at the Department of Gynaecological Endocrinology at the University Hospital of Vienna. 159 patients could be included in the follow-up and interviewed by standardized questionnaire. They were treated by laparascopy (n=57), laparatomy (n=41) and/or hysteroscopy (n=61). We found following main indications for the operation: menometrorrhagia (n=105), dysmenorrhea (n=87), pain (n=76), menstruation disorder (n=51) and sterility (n=80). Multiple declarations were possible.

42 (53%) of 80 patients with sterility became pregnant after myomectomy, 24 women gave birth at term, 8 times by vaginal delivery and 16 times by cesarean section. 18 pregnancies were lost by abortion. Totally 88% of the operated patients declared, that their subjective satisfaction with the operation outcome is very good.

Conclusions:

The long-term outcome of myomectomies is very good. A high improvement of symptoms and subjective satisfaction could be shown. The pregnancy rate after myomectomy in patients with sterility is very promising. Myomectomy is an effective therapy in these patients after exlusion of other sterility causes.
