Shared Decision Making und Patientenzentrierung - vom Paternalismus zur Partnerschaft in der Medizin

Zusammenfassung

Patienten erwarten von ihren Ärzten in zunehmendem Maße mehr Informationen und mehr Beteiligung an Entscheidungen, die sie betreffen. Shared Decision Making ist das am weitesten entwickelte Konzept einer patientenzentrierten Arzt-Patient-Kommunikation. Im ersten Teil dieses Aufsatzes wird das Shared-Decision-Making-Modell im Zusammenhang mit herkömmlichen Modellen der Arzt-Patient-Beziehung beschrieben. Die Haltung der Ärzte und der Patienten zur Patientenbeteiligung wird dargestellt. Im zweiten Teil geht es um die Risikokommunikation und die Verlässlichkeit der medizinischen Wissensbasis als wesentliche Elemente von Shared Decision Making. Abschließend werden die Auswirkungen von Shared Decision Making auf die Behandlungswahl und auf die Behandlungsergebnisse erläutert.

Abstract

Patients expect more information than they get and a higher degree of participation concerning decisions about their treatment. Shared decision-making is the most advanced model of patient-centred care. The first part of this two-part series offers a description of the shared decision-making model in comparison to traditional models of doctor-patient communication and an account of the attitude of doctors and patients towards sharing decisions. The second part deals with risk communication and the validity of the knowledge base of medicine as prerequisites for shared decision-making. Finally, the consequences of shared decision-making for treatment decisions and treatment outcomes are delineated outlined.
