Primary Peripheral T-Cell Lymphoma of the Central Nervous System

Abstract
A 47-year-old man was admitted to our hospital with a nine-week history of visual disturbance, headache, disorientation and facial nerve palsy. Serial cranial magnetic resonance imaging (MRI) revealed progressive bilateral occipital edema with hemorrhage and meningeal involvement. There were no hints of hereditary or acquired immunodeficiency. Laboratory examination for bacterial and viral causes was negative. Open brain biopsy revealed primary central nervous system lymphoma of the extraordinary rare so-called “peripheral” T-cell type. The further course was fatal; the patient died 10 weeks after the onset of symptoms from tumor progression before planned chemotherapy could be started.
Conclusion: If primary central nervous system lymphoma (PCNSL) is suspected, brain biopsy - either open biopsy or stereotactic biopsy - should be performed straight away to enable a rapid start of chemotherapy and/or radiotherapy. Peripheral T-cell lymphoma was highly aggressive in this case leading to the patient's death within several weeks.
Zusammenfassung
Ein 47-jähriger Mann klagte über seit neun Wochen bestehende Kopfschmerzen, Sehstörungen und faziale Schwäche. Bei der Aufnahme war er desorientiert. Magnetresonanzuntersuchungen im Verlauf hatten ein progredientes bilaterales okzipitales Hirnödem ergeben mit Einblutungen und Beteiligung der Dura mater. Es gab keine Hinweise auf das Vorliegen einer angeborenen oder erworbenen Immunsuppression, virologische und bakteriologische Tests waren negativ. Bei der offenen Hirnbiopsie wurde ein sehr seltenes intrakranielles peripheres T-Zell Lymphom nachgewiesen. Der weitere Verlauf war fulminant; der Patient verstarb 10 Wochen nach Einsetzen der Symptome noch bevor die geplante Chemotherapie appliziert werden konnte.
Schlussfolgerung: Bei Verdacht auf ein intrakranielles Lymphom sollte zügig eine offene oder stereotaktische Hirnbiopsie angestrebt werden, damit unverzüglich eine chemotherapeutische und/oder strahlentherapeutische Behandlung zum Einsatz kommen kann. Das sehr seltene periphere T-Zell-Lymphom hat in diesem Fall einen rasch progredienten malignen Verlauf genommen und zum Tode des Patienten innerhalb einiger Wochen nach Beginn der Symptome geführt.
