Gastroschisis with Omphalomesenteric Artery Remnant, Colonic Atresia and Arthrogryposis Multiplex Congenita

Abstract

The pathogenesis of gastroschisis (GS) is controversial. We present a case of GS complicated by colonic atresia and arthrogryposis multiplex congenita. This association is attributed to intrauterine vascular compromise. Furthermore, a probable omphalomesenteric artery (OMA) remnant was found on the right side of the defect opposite the umbilicus and extended with bifurcation to the mesenteries of the colon and ileum, between which colonic atresia occurred. This provided further evidence in support of the hypothesis that GS is caused by an intrauterine interruption of OMA.

La pathophysiologie du gastroschisis est controversée. Nous présentons un cas de gastroschisis compliqué par une atrésie du côlon et une arthrogrypose congénitale multiple. Cette association est attribuée à une anomalie vasculaire intra-utérine. A droite du défaut, en face de l'ombilic, un reliquat probable de l'artère omphalomésentérique (AOM) présentant une bifurcation pour les mésentères du côlon et de l'iléon de part et d'autre de l'atrésie. Ces observations sont en faveur d'une interruption intra-utérine de l'AOM comme cause du gastroschisis.

Resumen

La patogenia de la gastrosquisis (GS) es controvertida. Presentamos un caso de GS complicada por atresia del colon y artrogriposis múltiple congénita. Esta asociación se atribuye a compromiso vascular intrauterino. Además se encontró lo que parecía un resto de la arteria ofalomesentérica (OMA) en el lado derecho del defecto al otro lado del ombligo que se extendía con la bifurcación del mesenterio del colon y del ileon entre los que se había producido la atresia colónica. Esto aporta nueva evidencia para mantener la hipotesis de que la GS es debida a la interrupción intrauterina de la OMA.

Zusammenfassung

Die Pathogenese der Gastroschisis wird kontrovers diskutiert. In der vorliegenden Arbeit wird ein Kind mit Gastroschisis vorgestellt, bei dem gleichzeitig eine Kolonatresie, eine Arthrogryposis multiplex congenita sowie ein Residuum einer Arteria omphalomesenterica auf der rechten Seite gefunden wurden. Diese embryonale Arterie fand sich gegenüber dem Nabel und erstreckte sich von der Bifurkation zum Mesenterium der atretischen Segmente des Kolons und Ileums. Der vorliegende Fall ist interessant, da er wiederum die Hypothese unterstützt, dass die Gastroschisis durch eine intrauterine Unterbrechung der Arteria omphalomesenterica bedingt sein könnte.
