Zusammenfassung

Ein möglichst exaktes prätherapeutisches Staging ist die Voraussetzung für moderne multimodale Therapiekonzepte bei gastrointestinalen Tumoren. Auch die modernsten bildgebenden Verfahren weisen noch nicht die erforderliche diagnostische Genauigkeit auf, so dass die Rolle der erweiterten diagnostischen Laparoskopie als ergänzendes Verfahren untersucht wird. Nach der bisherigen Datenlage ist der Wert der zusätzlichen diagnostischen Laparoskopie je nach Tumorentität unterschiedlich zu bewerten: Bei fortgeschrittenem Adenokarzinom der distalen Speiseröhre und beim Magenkarzinom sind in 20 bis 40 % der Fälle Zusatzinformationen zu erwarten, so dass der Einsatz insbesondere vor geplanter multimodaler Therapie sinnvoll erscheint, jedoch nicht dagegen beim Plattenepithelkarzinom der Speiseröhre. Lohnend scheint die EDL auch bei fortgeschrittenen primären und sekundären Leberläsionen zu sein, während die Rolle beim Pankreaskarzinom eher als gering eingeschätzt wird.
