Abstract

Zusammenfassung

Grundlage für das Verständnis der Pharmakologie der α-Blocker im männlichen Urogenitaltrakt ist die Kenntnis neurophysiologischer Aspekte der Sexualfunktion. Bei der komplexen autonomen Steuerung übt der Sympathikus über α-Adrenozeptoren (α-AR) vorwiegend eine inhibitorische Kontrolle aus. Der Transmittereffekt der Katecholamine an der glatten Muskulatur von Schwellkörper und penilen Gefäßen führt durch Tonuserhöhung und Kontraktion zur Detumeszenz. Die parasympatische Erektion wird an cholinergen und vor allem an NANC-Synapsen (Non-Adrenerg-Non-Cholinerg) über NO und VIP (Vasoaktives Intestinales Polypeptid) vermittelt. Ein Netzwert aus Feedback-Steuerung, gegenseitiger Hemmung und gemischt-afferent-efferenter Verschaltung via spinale Reflexbogen (Erektions-, Ejakulations-Zentrum) reguliert den jeweiligen Funktionszustand (-ablauf). Als übergeordnete Schaltstelle obliegt dem Hypothalamus die zentralnervöse Integration, Koordination und Initiierung sexueller Funktionen. Während die Erforschung der peripheren Rezeptortopologie und Subtypisierung (α_1A/B/C etc.) fortschreitet, ist die Kenntnis der zentralen Mechanismen sehr lückenhaft. Die pharmakologischen Therapiemöglichkeiten sind mit Ausnahme des ersten α_1A/(D)-Blockers Tamsulosin in der BPH-Therapie relativ uniform. Einigen α₁-selektiven Blockern mit „balancierter” Subspezifität kann ein proerektiler Effekt nachgewiesen werden, wodurch der positive QoL-Effekt beim LUTS-Patienten gesteigert werden kann. Tamsulosin soll dagegen aufgrund einer hohen Inzidenz einer Ejakulationsstörung die sexuelle Zufriedenheit negativ beeinflussen. In der Behandlung der erektilen Dysfunktion hat der nicht-selektive α-Blocker Phentolaminmesylat (p. o.) bei älteren Patienten eine signifikante (bis marginale) Wirkung, während Yohimbin (α₂-Blocker) als zweifelhaft bis gefährlich eingestuft wird. Die genauen Wirkmechanismen (z. B. zentral vs. peripher) sind auch hier noch nicht hinreichend bekannt. Optionen zum Einsatz (und zur Weiterentwicklung) von α₁-Blockern bei simultanen internistisch-urologischen Problemen sollten trotz limitiertem Indikationsspektrum geprüft werden.

Abstract

Understanding of the pharmacology of α-blockers in the male urogenital tract is based on the knowledge of neurophysiologigal aspects of sexual function. In the complex autonomic control system, the nervus sympathicus effects a mainly inhibitory control via α-adrenoceptors (α-AR). Via increased tone and contractility, the transmitting effect of catecholamines along the smooth muscles of the spongy bodies and penile vessels results in detumescence. Parasympathetic erection is transmitted at cholinerg and mainly at NANC-synapses (Non-Adrenerg-Non-Cholinerg) via NO und VIP (Vasoactive Intestinals Polypeptide). A network of feedback-control, mutual inhibition and mixed afferent-efferent switches via spinal reflex arcs (erection-, ejaculation-centre) regulates the respective state (process). Acting as a superior relay, the hypothalamus integrates, coordinates and initiates sexual functions. While progress has been made regarding research on the peripheral receptor topology and knowledge of subtypes (α_1A/B/C etc.), the knowledge of central mechanisms is comparatively incomplete. With the exception of the first α_1A/(D)-blocker tamsulosine, pharmacological therapy options are relatively uniform. In several α₁-selective blockers with ‘balanced’ subspecifity, a proerectile effect can be confirmed, leading to an increased positive QoL-effect in LUTS patients. Whereas tamsulosine, due to a high incidence of ejaculation disorders, seems to have a negative influence on sexual satisfaction. In the treatment of erectile dysfunction, the non-selective a-blocker phentolaminmesylate shows a significant (to marginal) effect in elder patients, whereas Yohimbin (α₂-blocker) is considered to have a doubtful to dangerous effect. Here, too, the exact mechanisms (e. g. central vs peripheral) are not sufficiently known. Options for the use (and further development) of α₁-blockers in problems of a simultaneously internal medical and urological nature are to be carefully studied despite the limited spectrum of indications.
