Anamnese und klinischer Befund: Eine 30-jährige Patientin, die als Erstgravida nach komplikationslosem Schwangerschaftsverlauf ein 3340 g schweres Kind entbunden hatte, bemerkte kurz nach unauffälligem Wochenbettverlauf einen nicht rückläufigen Bauchumfang.
Diagnostik: Sonographisch wurde ein großer retroperitonealer Tumor festgestellt. CT und MRT wiesen einen vom unteren Pol der rechten Niere ausgehenden großen retroperitonealen verdrängenden tumorösen Prozess nach.
Therapie und Verlauf: Unter der Verdachtsdiagnose eines renalen Liposarkoms erfolgte die Exstirpation des Tumors in toto unter Einschluss der rechten Niere sowie die Exstirpation von präkaval und präaortal gelegenen Lymphknoten. Makroskopisch zeigte sich ein ca. 3,2 kg schwerer und 27 × 19 × 10 cm großer myxoider Tumor. Nach eingehender histologischer Untersuchung des Operationspräparates wurde die Diagnose eines retroperitoneal gelegenen Angiomyolipoms gestellt.
Folgerung: Die postpartale Diagnose eines gigantischen retroperitoneal gelegenen Angiomyolipoms mit Lymphknotenbefall stellt einen besonders seltenen Befund dar, der in dieser Form noch nicht in der Literatur beschrieben wurde.
History and presenting complaint: A 30-year-old primipara after a normal pregnancy had delivered a 3340 g child. After an uneventful post-partum period she had noticed her abdomen failing to reduce in size.
Investigations, diagnosis and treatment: The abdominal sonography discovered a large retroperitoneal tumor. CT and MRI showed a giant tumor which originated from the right kidney. Suspecting the diagnosis of renal liposarcoma the kidney and tumor were excised with removal of enlarged precaval and preaortal lymph nodes. Gross inspection revealed a ca. 3,2 kg myxoid tumor, measuring 27 × 19 × 10 cm. The histological examination of the surgical preparation revealed a retroperitoneal angiomyolipoma.
Conclusion: This is the first case of a giant retroperitoneal angiomyolipoma with lymph node involvement diagnosed post partum.
