The author reported his experience since 1995 with reconstructions of unicondylar defects of the tibial condyle in children, using the proximal fibular head with transfer of articular cartilage. Two methods of transfer were used. A peroneal artery pedicled ipsilateral fibular head transposition to a defective lateral tibial condyle defect demonstrated a poor prognosis, with gradual absorption of the transposed fibular epiphysis. A free vascularized fibular head transplantation with microvascular anastomosis was used in a case with medial condyle defect of the tibia and demonstrated very satisfactory results. The transplanted fibular head grew well, with adaptive changes in the knee joint. There was no growth disturbance or joint motion limitation during a 4-year follow-up.
