Zusammenfassung

Wundheilung und Wundbehandlung nehmen einen zentralen Stellenwert im chirurgischen Alltag ein. Ziele des Artikels sind die aktuelle Darstellung pathophysiologischer Abläufe der Wundheilung und darauf basierend die Darstellung klinischer Behandlungsstrategien. Das volkswirtschaftlich brisante Thema chronische Wunde wird anhand der drei häufigsten Krankheitsentitäten - diabetisches Fußsyndrom, Ulcus cruris venosum und Ulcera cruris bei peripherer arterieller Verschlusskrankheit - diskutiert. Die wissenschaftlich gesicherten Eckpfeiler des modernen Wundmanagements mit feuchter Wundbehandlung, standardisierter chirurgischer Lokalbehandlung und optimaler orthopädie-technischer Versorgung der Patienten werden dargestellt.

Abstract

Wound healing and wound care play a pivotal role in surgical treatment. Only a century ago, complicated and incomplete healing after injury and operations was the rule rather than the exception. Recent advances in cellular and molecular biology have greatly expanded the knowledge of the biological processes involved in wound healing and tissue regeneration and have led to improvements in wound care. The goal of this article is to summarize modern concepts of the biology of wound healing and standards of optimal wound treatment. The most frequent scenarios of chronic wound healing are characterized. Modern wound management based on moist wound dressings, standardized local surgery and optimized off-loading devices is discussed.
