ABSTRACT

The inverted T-scar remains the gold standard of breast reduction. The senior author (D.H.L.) used this technique for the first 12 years of his practice. He has since discovered that the vertical portion of the T-scar is not necessary to achieve the same pleasing breast shape. In fact, he now understands that a better periareolar scar can be achieved as well with the no vertical scar technique. This is why he has used this method for most of his breast reductions for the last 5 years in over 130 cases. The no vertical scar technique described in this article is very similar to the Wise pattern inferior pedicle T-scar breast reduction most commonly performed today. Because of this, most surgeons will easily be able to make the few modifications required to eliminate the vertical portion of the T-scar.
