Therapie der Induratio penis plastica mittels Stoßwelle:Eine kritische Literaturübersicht

Zusammenfassung

Die Induratio penis plastica stellt eine ideopathische Erkrankung der Tunica albuginea dar. Von C. Bellorofonte wurde die Stoßwellenbehandlung der IPP erstmals 1989 beschrieben. In der Folgezeit sind bis heute insgesamt 5 Arbeiten medline-relevant und eine Arbeit in der Aktuellen Urologie publiziert worden. Allen diesen Studien ist gemein, dass es sich um nicht randomisierte Studien mit einem inhomogenen Patientengut und nicht standardisierten Behandlungsschemata handelt. Die Ergebnisse, bezogen auf den Schmerzrückgang, reichen von 60 % Schmerzrückgang bis zu 100 % schmerzfrei, und bezogen auf die Deviation von 11 % bis 78,5 %, bis zu 17 % keine Deviation. Die Ergebnisse zur Verbesserung der Fähigkeit zum Geschlechtsverkehr reichen von 42 % bis zu 74,4 %. Bei kritischer und vorsichtiger Betrachtung lässt sich aus den vorliegenden Arbeiten ableiten, dass es sich bei der ESWT um eine nebenwirkungsarme Behandlungsmethode handelt, die den Plaque nicht zuverlässig entfernt und deren analgetischer Effekt mehrfach belegt wurde. Hinsichtlich der Verbesserung der Deviation kann gefolgert werden, dass diese im instabilen Stadium nicht, jedoch im stabilen Stadium im statistisch nicht signifikantem Bereich belegt werden konnte. Die GV-Fähigkeit der Patienten wurde subjektiv verbessert. Basierend auf diesen Ergebnissen muss die ESWT der IPP, wie viele andere Therapieverfahren, zum gegenwärtigen Zeitpunkt als fragwürdig gelten. Daher erscheint eine multizentrische, wenn möglich plazebokontrollierte Studie mit Symptomobjektivierung als erforderlich, um den Stellenwert dieser Behandlung näher definieren zu können.

Abstract

The induratio penis plastica is a ideopathic disease of the tunica albuginea. C. Belerofonte described first the shock wave treatment of the induratio penis plastica in 1989. In the following period of time 4 medline relevant papers and one paper in the Aktuelle Urologie were published. All this studies have in common, that they are not randomized and that the patient groups are inhomogenous. Furthermore there exists up to now no standardized treatment protocoll. The results regarding erection pain are reacing from 60 % pain reduction to 100 % painfree. In terms of deviation the results reaches from 11 % improved to 78.5 % improved and to 17 % total deviation realise. The results regarding the improvement of the possibility having sexual intercourse are reaching from 42 % to 74.4 %. Having a critical look onto this data the ESWT seem to be a treatment method with minimal side effects, which dose not reduces the plaque size but the erection pain. Focussing on deviation the ESWT seems to have a positive influence, however this is not static significant. The possibility of having sexual intercause is subjectively improved after ESWT. Based on this results the ESWT can up to now not be recomended for the treatment of IPP. In order to define the role of ESWT for IPP a randomized, placebo controlled study is requested.
