Supra-/diakondyläre Frakturen des distalen Humerus beim geriatrischen Patienten

Zusammenfassung

Die distale Humerusfraktur des älteren Menschen mit osteoporotischen Knochenverhältnissen ist eine im Vergleich zu den proximalen Humerusfrakturen eher seltene Verletzung. Aufgrund der schlechten Ergebnisse der konservativen Therapie ist die operative Stabilisierung dieser Frakturen die Therapie der Wahl. Wegen der oft ausgeprägten osteoporotischen Veränderungen sind bei der Osteosynthese einige Besonderheiten zu beachten. Hierzu zählt z. B. die Verwendung längerer Platten mit der Möglichkeit, mehr Schraubenfixpunkte zu verankern. Auch sollten alternative Osteosyntheseverfahren wie die Zuggurtungsosteosynthese am radialen und ulnaren Pfeiler mit geringerer Devastierung der ossären Durchblutung berücksichtigt werden. Zusätzlich zur Beschreibung der OP-Technik und der Ergebnisse der operativen Therapie gehen wir nicht zuletzt auf die Grenzen der Osteosynthese der distalen Humerusfrakturen des alten Menschen ein und auf das Vorgehen in einem solchen Fall.

Fractures of the Distal Humerus in the Elderly

Fractures of the distal humerus in elderly people with osteoporotic bone are rare compared to the common fractures of the proximal humerus. Due to the bad functional results of conservative treatment - the osteosynthesis of the distal humerus is the method of choice in these fractures. There are special rules that have to because of the often very severe osteoporotic changes special rules apply to the osteosynthesis. This is i. e. the implantation of longer plates with more screws to increase stability. Other forms of osteosynthesis should be considered, too, i. e. the double tension band osteosynthesis on the ulnar and radial column with only little devastation of the blood supply to the bone. In addition to the description of the technique of the operative treatment and of the results, we do describe the limitation of the osteosynthesis of the fractures of the distal humerus in elderly people and the workaround in these cases.
