Die Rolle bildgebender Verfahren bei neuromuskulären Erkrankungen

Die wichtigsten Untersuchungen bei Verdacht auf eine neuromuskuläre Erkrankung erfassen die Funktion des Muskelparenchyms (Muskelfasern). So vermittelt die Kraftprüfung einen Eindruck über den Anteil funktionsfähiger Muskelfasern (optimale Mitarbeit des Patienten vorausgesetzt), die Aktivität der Muskelenzyme (Kreatinkinase und Aldolase) im Serum über die Intaktheit der Muskelfasermembranen bzw. den Untergang von Fasern und die Elektromyographie (EMG) über die elektrische Funktion der Fasern. Das Muskelmesenchym sowie seine Veränderungen (intramuskuläres Bindegewebe, Fettgewebe, Verkalkungen, Muskelödem, Blutungen, Amyloidablagerungen usw.) entgehen diesen Methoden nahezu völlig. Lediglich die histopathologische und mit erheblichen Abstrichen die klinische Untersuchung (Muskelmasse und -konsistenz) und bildgebende Verfahren informieren hierüber.
