Erythema anulare centrifugum as leading symptom of a non-small cell lung cancer.

A 72-year old male smoker was referred to our clinic due to figurate erythemas lasting since 10 months. The patient was in good general condition, except for a body weight loss of 5 kilo during the last 4 months. There were annular and polycyclic, partially confluent erythemas with central clearing on the trunk and extremities. A skin biopsy showed the typical histopathological features of erythema annulare centrifugum. Further examinations disclosed a non-small cell lung cancer in stage IIIB (UICC). After treatment with fractionated irradiation, the tumour size decreased and the skin leasions disappeared. Four month later tumour progression was accompainied by reappearance of figurate erythemas. Chemotherapy with cis-platinum and etoposide resulted in both significant reduction of the tumour and healing of the skin leasions. However, 13 months later, the patient suffered from marked and finally fatal tumour progression with brain metastases that was again associated with the development of erythemas. In conclusion, the erythema annulare centrifugum belongs to the facultative paraneoplastic syndromes of the skin, and can lead not only to the diagnosis of an internal malignancy, but may also help to monitor the course of the disease.
