Die Plattenosteosynthese bei proximaler 4-Fragment-Humerusfraktur über den erweiterten Deltoid-Split-Zugang

Zusammenfassung

Als „Goldstandard“ bei der operativen Therapie proximaler Humerusfrakturen hat sich der deltoideopektorale Zugang etabliert. Wir stellen in einem OP-Video als Alternative den erweiterten Deltoid-Split-Zugang vor. Hierbei erfolgt eine direkte laterale Inzision unter Darstellung und Schonung der vorderen N.-axillaris-Anteile. Hierüber ist nach unserer Einschätzung eine bessere Visualisierung insbesondere der Tuberculum-majus-Fragmente und eine einfachere Plattenpositionierung möglich. Klinisch fassbare neurologische Schäden werden im eigenen Krankengut und auch nach der Literatur nicht beschrieben.

Abstract

The deltoideopectoral approach is established as the gold standard in the surgical treatment of proximal humeral fractures. As an alternative, we demonstrate the extended deltoid approach with an intraoperative video. A direct lateral incision is performed and the anterior parts of the axillary nerve are identified and preserved. In our experience, this approach allows improved visualisation of the greater tuberosity and easier positioning of locking plates. Clinically relevant neurological injuries cannot be seen in our patients or in the literature.
