Biliary Ablation for Sectoral Duct Injury to Avoid Surgery

Purpose: This study aimed to present a case of sectoral duct injury post cholecystectomy managed by percutaneous drainage and chemical biliary ablation.

Materials and Methods: A 23-year-old female presented with bile leak secondary to sectoral duct injury post cholecystectomy. The patient was referred to interventional radiology for a temporary percutaneous transhepatic biliary drainage (PTBD), followed by right hepatectomy, given the site and extent of injury. However, the patient was managed by PTBD, followed by chemical bile duct ablation using absolute alcohol.

Results: The treatment was concluded after eight sessions when there was no more output from the PTBD. The PTBD was removed after 5 months without complications. The patient is doing well for the past 2 years and no surgery was performed.

Conclusion: Chemical biliary ablation can help induce biliary fibrosis to prevent biliary leak, recurrent cholangitis, and avoid further surgeries.
