Prediction of long-term improvement of insulin sensitivity after weight loss by adipose tissue ANP system and modification by a 12-months weight maintenance intervention: A randomized controlled trial

Objective:

Weight loss by lifestyle intervention lacks long-term efficacy. Weight regain diminishes metabolic improvements. Sustainable weight loss interventions are therefore sought after. We investigated the effect of a 12-months weight maintenance intervention on body weight and metabolic improvements and the role of ANP system.

Methods:

156 obese participants (age > 18 years, BMI ≥27 kg/m2) underwent a 12-weeks weight loss program. Those who lost > 8% body weight (143 participants) were randomized to 12-months weight maintenance intervention or control. Intervention ended after 12 months (T12). Weight and insulin sensitivity were measured yearly for 3 years (T48). We analyzed ANP system in adipose tissue.

Results:

BMI reduced by 4.67 ± 1.47 kg/m2 after weight loss. The weight maintenance intervention inhibited weight regain until T12. There was no effect on BMI from T18 to T48. While BMI at T36 and T48 was lower than baseline, HOMA-IR was not different from baseline at T36 (-0.49 (95% CI, -1.00 to 0.02)) or T48 (0.14 (95% CI, -0.64 to 0.36)). GlucoseAUC during oral glucose test returned to baseline at T24. HOMA-IR and glucoseAUC were different between groups at T12 but not afterwards. Adipose tissue NPR-C mRNA expression after weight loss predicted weight regain (r = 0.398; p < 0.05) while NPR-A mRNA expression predicted HOMA-IR (r = -0.422; p < 0.05).

Conclusions:

We found no benefit of a 12-months weight maintenance intervention after weight loss on weight regain and metabolic improvement. Adipose tissue ANP expression might be involved in long-term weight control and insulin sensitivity.
