EFFICACY OF SPYGLASS-GUIDED ELECTROHYDRAULIC LITHOTRIPSY (EHL) IN DIFFICULT BILE DUCT STONES

Aims:
We aimed to evaluate the efficacy and safety of Spyglass-guided electrohydraulic lithotripsy (EHL) in treating common bile duct stones (CBD) not amenable to conventional endoscopic therapy “difficult CBD stones”, in a single center.
Methods:
All patients underwent Spyglass-guided EHL from August to November of 2017.
Results:
A total number of 9 patients underwent Spyglass-guided EHL. The majority (65%) of them were females. The mean age was 67,5 years (from 21 to 85). Bile duct clearance was achieved in all of them (100%). Eighty-nine percent required only one ERCP to clear the CBD and 11% required two ERCPs. Only in one patient there was an elevation of amylase to 1000 without pain. In one male patient 79 years old (11%) there was a diagnosis with the spyglass of a very small cholangiocarcinoma below the bile duct stone.
Conclusions:
Spyglass-guided EHL is a very effective procedure in treating “difficult CBD stones”.
