Zusammenfassung

Neurostimulationsverfahren zur Epilepsietherapie haben in den vergangenen Jahren eine rasche Weiterentwicklung erlebt. Neben der Vagusnervstimulation liegen nun auch für die tiefe Hirnstimulation des Nucleus anterior thalami und die responsive Fokusstimulation positive Ergebnisse aus Phase-III-Studien vor. Mit den transkutanen Stimulationsverfahren der externen trigeminalen Stimulation und der transkutanen Vagusnervstimulation stehen zudem auch nicht invasive Stimulationsmethoden zur Verfügung, welche jedoch noch einer weiteren klinischen Prüfung bedürfen. Der vorliegende Artikel soll die einzelnen Stimulationsverfahren beleuchten und ihren Einsatz in der Epilepsiebehandlung einordnen.

Summary

Neurostimulation in epilepsy underlies a rapid evolutional process. Positive results from randomized controlled trials are now available for vagus nerve stimulation, deep brain stimulation of the anterior thalamus and responsive neurostimulation. Transcutanous stimulation of the vagal and trigeminal nerve can be applied as first non-invasive stimulative methods but need further evidence from clinical trials. This article reviews mechanisms of action, efficacy and safety of neurostimulation devices in treatment of drug-resistant epilepsy.
