Psychogener Husten

Zusammenfassung

Der psychogene Husten als Ursache des chronischen Hustens im Kindes- und Jugendalter wird mit einer Häufigkeit von 1–4 % angegeben, mit einem Altersgipfel zwischen 6 und 14 Jahren. Die Hustenanfälle sind eruptiv und unproduktiv. Die Patienten selber leiden durch den Husten, im Gegensatz zu ihrer Umwelt, wenig. Für die Diagnosestellung ist entscheidend, dass Kinder mit psychogenem Husten im Schlaf nicht husten. Körperliche Untersuchung und Lungenfunktion bzw. Röntgen-Thorax erbringen keinen pathologischen Befund. Therapeutisch gilt es zunächst, die Eltern darüber zu informieren, dass dem Geschehen keine organische Ursache zugrunde liegt. Die Prognose des psychogenen Hustens ist in den allermeisten Fällen immer exzellent. Gewöhnlich endet die Symptomatik, nachdem den Eltern mitgeteilt wurde, dass es sich um eine Angewohnheit oder einen Tic handelt. Selten muss das Kind in psychotherapeutische oder psychiatrische Behandlung.

The prevalence of psychogenic cough in childhood and adolescence is around 1–4 %, with a peak between 6 and 14 years. Psycho-genic cough is harsh and explosive, very disturbing and never occurs during sleep. The child is usually not particularly worried about the symptom. Physical and radiographic examinations of the respiratory tract and microbiological investigations are normal. The long term outcome is almost always excellent for the patient with a psychogenic cough. Usually the cough will resolve after the parents are reassured that there is no underlying disease and that it is simply a habit. Rarely, referral for psychiatric consultation has to be considered.
