Botulinumtoxin in der Urologie

Zusammenfassung

Alle Indikationen zur Anwendung von Botulinumtoxin A (BoNT A) in der Urologie befinden sich im Status des sogenannten off label use, entsprechend sind Kernfragen wie z. B. die Kostenübernahme nicht geklärt. Erst 20 Jahre nach der ersten Anwendung in der Urologie werden Zulassungsstudien durchgeführt. Andererseits sind insbesondere die Behandlungsmöglichkeiten im Bereich der neurogenen Harnblase so etabliert, dass sie bereits in die urologischen Leitlinien Einzug gefunden haben. Hinsichtlich der Dosierung von BoNT A und der optimalen Anwendungstechnik bestehen weder allgemein anerkannte Handlungsanweisungen noch offizielle Empfehlungen. Dies gilt auch im Bereich der Anästhesie zur Injektion von BoNT A im urologischen Gebiet. Nicht wenige Patienten schrecken davor zurück, sich regelmäßig, zum Teil jährlich, einer Allgemeinanästhesie zu unterziehen. Ein besonderer Hoffnungsschimmer stellt für multimorbide Patienten mit hohem OP-Risiko die sich entwickelnde Behandlung der Prostatahyperplasie mittels BoNT A dar, insbesondere da in allen Studien die Lokalanästhesie angewandt wurde und nur wenige Nebenwirkungen auftraten.

Summary

Twenty years after the first use of Botulinum toxin A (BoNT A) in urology first controlled studies for approval in the different indications are carried out or finished recently. Therefore all indications for the use of BoNT A in urology are still in the status of an off label use. On the other hand BoNT A treatment of neurogenic bladder is well established in clinical practise and the use of BoNT A is recommended and part of urological guidelines in Germany. Up to now there are no official recommendations for optimal dosing and application of BoNT A in the different urological indications. Also there are no general instructions for anaesthesia for the different procedures used for BoNT A application in urology. Today still some patients worried about a general anaesthesia twise a year and avoid therefore a sufficient treatment with BoNT A. Corresponding the question of reimbursement is open. A glimmer of hope for multimorbide patients with high operation risk is the use of BoNT A in the treatment of prostate hyperplasia All controlled studies using local anaesthesia and local BoNT A injections showed low frequencies of side-effects and good treatment result for BoNT A injection in the prostate.
