Zusammenfassung

Die Substitutionsbehandlung Drogenabhängiger ist eine der realistischen therapeutischen Optionen. Ihre Umsetzung scheitert häufig an Unkenntnis der rechtlichen Grundlagen und der praktischen Durchführung, die dargestellt werden. Mittlerweile stehen mit Methadon, L-Methadon und Buprenorphin verschiedene Substitutionsmittel zur Verfügung mit möglicherweise differenziellen Indikationskriterien. Wichtig ist auch die Mitbehandlung körperlicher Begleitererkrankungen, speziell HIV und Hepatitis. Aktuelle Indikationsempfehlungen werden berichtet

Summary

Substitution therapy of drug dependent patients is one of the realistic treatment options but often limited by legal concerns and practical problems. Meanwhile there is a number of different pharmacological agents available including methadone, l-methadone and buprenorphine. The treatment of concomitant physical disorders including HIV and hepatitis is of great relevance. Possible pharmacological strategies are discussed.
