Zusammenfassung

Hintergrund: Viele pädiatrisch eingesetzte Arzneimittel sind für Kinder nicht zugelassen. Der Mangel an Informationen zu Anwendung und Nebenwirkungen ist in der Frühgeborenen- und Intensivmedizin besonders hoch.

Ziel: Das Ziel bestand in der Untersuchung des Zulassungsstatus der Medikamentenverordnungen an einer universitären Kinderklinik zur Priorisierung und Planung von Arzneimittelstudien.

Material und Methoden: Auf einer interdisziplinären, einer neonatologischen Intensivstation und einer allgemeinpädiatrischen Station wurden über einen Zeitraum von 3 Monaten alle Arzneimittelverordnungen erfasst. Zur Analyse des Zulassungsstatus dienten die „Rote Liste®” und die Fachinformationen.

Ergebnisse: 377 Patienten erhielten 2486 Verordnungen. Die häufigsten Arzneimittel waren Paracetamol, Vitamin K und Acetylcystein (ACC). Unlizensiert waren 5,5% aller Verordnungen; 39,5% wurden außerhalb der Altersgrenzen, 16,7% mit nicht zugelassener Indikation angewendet.

Schlussfolgerung: Die unlizensierte und Off-Label-Arzneimittelanwendung ist noch weit verbreitet. Durch systematische Analysen können in Zusammenarbeit mit EMEA (European Medicines Agency) und PAED-Net Prioritätenlisten für Studien erstellt werden, die die Arzneimittelsicherheit erhöhen.

Summary

Background: Many drugs prescribed for children are used in an off label or unlicensed manner. The lack of information is particularly high in neonatology and intensive care medicine.

Aim: To analyze the license status of drug prescriptions in a monocentric survey in order to objectively evaluate the need for drug trials in paediatrics.

Material and methods: Drug prescriptions for inpatients on a neonatological and an interdisciplinary intensive care unit as well as on a general paediatric ward were collected for a period of 3 months. Analysis for license status was performed using information from the Rote Liste® and the SmPCs (Summary of product characteristics).

Results: 2486 drugs were prescribed to 377 patients. The most commonly prescribed drugs were paracetamol, vitamin K and N-acetyl cysteine. 5.5% of all drugs were unlicensed, 39.5% were used for patients beyond the licensed age range and 16.7% were used for an off label indication.

Conclusions: The use of unlicensed and off label drugs in paediatric medicine is wide spread. In order to increase the safety of paediatric drug prescriptions, international collaboration between paediatric networks (e. g. the German PAED-Net) and the EMEA (European Medicines Agency) is crucial.
