Peritonealdialyse in der Kleintierpraxis – Indikationen, Vorgehen, Komplikationen

Zusammenfassung

Ziel: Die Technik der Peritonealdialyse (PD) stellt eine Möglichkeit zur Therapie unterschiedlicher Erkrankung in der Kleintierpraxis dar. Ziel des vorliegenden Übersichtsartikels ist es, sowohl theoretische Grundlagen der PD als auch Indikationen, technische Aspekte der Behandlung und mögliche Komplikationen darzustellen. Datenquellen: veterinärmedizinische und humanmedizinische Literatur. Ergebnisse: Hauptindikation der PD stellt die Behandlung einer schweren Urämie dar. Daneben kann die Technik der PD zum Management verschiedenster Erkrankungen wie Elektrolyt- und Säure-Basen-Abweichungen, Uroabdomen, Peritonitis und Hypothermie eingesetzt werden. Die PD erfolgt über eine Dialysatlösung, die über einen Katheter ins Abdomen eingebracht und nach einer Einwirkzeit wieder abgelassen wird. Als häu-figste Störung treten Katheterflussprobleme auf, die wichtigste Komplikation ist eine Peritonitis. Schlussfolgerung: Trotz des hohen personellen und zeitlichen Aufwands und möglicher schwerer Komplikationen stellt die Peritonealdialyse eine auch in der tierärztlichen Praxis einsetzbare Technik dar, die sich vor allem zur Therapie einer schweren Urämie anbietet.

Summary

Objective: Technique of peritoneal dialysis (PD) is a tool for treatment of different diseases in small animal practice. Objective of the article is to point out theoretical basics, indications, technical aspects and potential complications of PD. Data sources: veterinary and human literature. Data synthesis: Main indication for PD is the treatment of severe uraemia. Additionally the technique can be used for management of different diseases as electrolyte- and acid base disorders, uroabdomen, peritonitis and hypothermia. During PD a dialysis solution is instillated into the abdomen via a catheter and released following dwell time. The most common failure is impaired flow of the solution through the catheter, the most common complication is peritonitis. Conclusion: Despite personal and time consumption and potential complications PD is a useful tool for therapy of severe uraemia in small animal practice.
