Zusammenfassung

Vorgestellt wird der Fall eines 76-jährigen Rheumatikers mit einer seit 21 Jahren dosisvariablen Dauertherapie mit Sulfasalazin, Chloroquin, Prednisolon und Methotrexat und endoprothetischem Ersatz beider Hüftgelenke und des linken Kniegelenkes sowie Wechselprothetik der linken Hüfte. Nach einem diaphysären Ermüdungsbruch mit erforderlichem totalem Femur zeigen die histologischen Befunde des Explantates in drei repräsentativen Schnittebenen die ungestörte Osteointegration eines Prothesenschaftes mit Oktaeder-Querschnitt und Mehrpunktabstützung in einem extrem geschwächten Knochenlager. Der hier gewählte Oktaederschafttyp mit Mehrpunktfixation und nicht intendierter endofemoraler Aussteifung durch erzwungenen vollflächigen Primärkontakt führt zur ungestörten Osteointegration.

Summary

This is the case of a 76-year-old rheumatic patient, who was treated for 21 years with variable dose long-term therapy with sulphasalazine, chloroquine phosphate, prednisone and methotrexate, with bilateral hip replacements and a left knee replacement, as well as an exchange prosthesis in the left hip. Following a diaphyseal fatigue fracture involving replacement of the whole femur, the histological findings in three representative sectional planes of the explant show undisturbed osteointegration of a prosthesis shaft with an octagonal cross-section and multiple-point support within an area of extremely weak bone. The octagonal-form shaft that was chosen here, with multiple-point fixation and unintentional endofemural reinforcement caused by the forced primary contact over the entire surface, resulted in undisturbed osteointegration.
