Zusammenfassung
Auffälliges Essverhalten ist bei adipösen Kindern und Jugendlichen weit verbreitet. Die Bandbreite von Essverhaltensmustern ist sehr groß. Im Rahmen des folgenden Beitrags sollen v.a. das emotionale Essverhalten sowie das Binge Eating (BE) als so genannte „enthemmte Essverhaltensmuster” näher betrachtet werden. Es mehren sich die Hinweise, dass diese Essverhaltensmuster nicht nur gehäuft bei adipösen Kindern und Jugendlichen zu beobachten sind, sondern auch eine wichtige Rolle in der Genese und Aufrechterhaltung der Adipositas spielen. Sie sind zudem mit einem erhöhten Auftreten von psychopathologischen Auffälligkeiten verbunden und können selbst wiederum Vorläufer einer klinisch-relevanten Essstörung sein. Eine umfassende Diagnostik des Essverhaltens von adipösen Kindern und Jugendlichen ist daher ebenso erforderlich wie eine Thematisierung des Essverhaltens innerhalb der multimodalen Therapie der Adipositas. Neben der Berücksichtigung von Gewichtsstatus und Lebensqualität sollte auch in Interventionsstudien die Veränderung des Essverhaltensmusters als Zielkriterium verstärkt untersucht werden.
Summary
Problematic eating behaviour is widespread among obese children and adolescents. The spectrum of eating behaviour patterns is very large. The following contribution sets a special focus on the so-called “disinhibited eating behaviour patterns”, like emotional and binge eating (BE) behaviours. There is increasing evidence that these eating patterns are not only frequently observed in obese children and adolescents, but also play an important role in the etiology and maintenance of obesity. These patterns are also associated with an increased incidence of psychopathological abnormalities and can even turn into predecessors of a clinically-meaningful eating disorder. A comprehensive diagnosis of the eating behaviour of obese children and adolescents is therefore just as necessary as the addressing of eating behaviour within the multimodal treatment of obesity. In addition to the consideration of weight status and quality of life, the change in eating pattern should be increasingly investigated as an outcome criterion in emerging intervention studies.
