Effektivität des Perforationsverschlusses mittels Over-The-Scope-Clip (OTSC) (in Abhängigkeit von der Lokalisation im Gastrointestinaltrakt)
Akute Perforationen des Gastrointestinaltrakts (primär oder sekundär) sind mit hoher Morbidität und Mortalität assoziiert. Ein endoskopischer Perforationsverschluss mittels Over-The-Scope-Clip (OTSC) kann eine Behandlungsalternative zur Chirurgie darstellen. Aktuell gibt es keine prädiktiven Faktoren für einen effektiven Perforationsverschluss mittels OTSC.
Wir präsentieren eine retrospektive Analyse von insgesamt 34 Patienten (16 weiblich, 18 männlich, 69,5 Jahre) mit 22 nicht-chirurgischen Perforationen (primär oder traumatisch, Gruppe 1) und 12 postoperativen Perforationen (z.B. Anastomoseninsuffizienzen, Gruppe 2). Ein vollständiger Perforationsverschluss wurde in 26/34 Patienten (76,5%) erreicht, 10/12 hiervon aus der postoperative Gruppe. In allen 6 Patienten mit einer Rektumperforation gelang der Verschluss (je 3 Gruppe 1 und Gruppe 2). In der Gruppe 1 konnten Perforationen im Magen (11/13) und Dünndarm (2/3) erfolgreich verschlossen werden. Perforationen im Oesophagus (2) und Kolon (1) konnten endoskopisch nicht verschlossen werden. In Gruppe 2 konnten hingegen die Perforationen im Magen (1), Duodenum (1), Jejunum (1), Ileum (1) und Kolon (1) erfolgreich behandelt werden. Der Perforationverschluss im Ösophagus gelang nur in 1 Fall (1/3) von Gruppe 2.
Prädiktiven positiven Faktoren für einen effektiven Perforationsverschluss sind postoperativen Perforationen, Lokalisation in Rektum, Magen oder postbulbären Duodenum. Die Lokalisation im Oesophagus hat eine negative Prognose.
