ESD in esophageal neoplasia
Background: In Europe Endoscopic Mucosal Resection (EMR) is the standard technique for treatment of benign and early malignant neoplasia of the esophagus despite the potential benefits of Endoscopis Submucosal Dissection (ESD). We report on our experience with the first 27 cases of ESD in the esophagus. Methods: From February 2012 to March 2015 all patients undergoing esophageal ESD were collected prospectively. Results: Out of 27 patients with esophageal ESD there was squamous cell neoplasia in 5, Barrett's neoplasia in 18 and subepithelial lesions in 4. Complete resection was achieved in 96%, en bloc resection in 92%. The complication rate was 15,3% with 3 cases of mild mediastinal emphysema (11,5%) and 1 delayed bleeding (3,8%). Additional esophagectomy due to sumucosal invasion was required in 1 patient. Conclusion: ESD in the esophagus is a safe and efficient technique for treatment of early esophageal neoplasia.
