Nigeria is known to have the highest twining rate in the world. In this report, we present a case of a woman MS, 32 years old, G4P4Ab, who had hysterotomy for conjoined twin (thoracopaque) and malformed triplet on 26/31/2015 at 21 weeks of gestation. There was no history of usage of teratogenic drugs and all previous deliveries were spontaneous vaginal. The ultrasound done on 24th March 2015 showed a viable co-joined twins, with thoraxes fused together but with separate active heart actions. The limps appeared to be just a pair, like in normal singleton and a non-viable mummified triplet. Placenta was anterior, polyhydramnion BPD 55 mm composite to gestational age as of 21 week and 6 day and EDD OF 22/7/15 (FIG1) the live babies co-joined twins died 3 minutes post delivery. The post operative phase was without complications and the mother was discharged home on the 30/3/15 and did not show up for post natal care. A review of the literature will be made. See figure 1
