Einleitung

Die Enteroskopie bietet die Möglichkeit, sowohl in der „single-balloon“- als auch mit der „double-balloon“-Technik Erkrankungen des Dünndarms zu diagnostizieren und ggf. interventionell-endoskopisch zu therapieren [1], [2], [3], [4], [5], [6]. In diesem Zusammenhang wurde für diese Methoden der Begriff der ballonassistierten Enteroskopie (BAE) geprägt. Bei Patienten nach vorangegangenen komplexen viszeralchirurgischen Oberbaucheingriffen mit Rekonstruktion mittels Y-Roux-Anastomose sowohl mit als auch ohne biliäre Rekonstruktion treten mitunter sekundäre pankreatobiliäre Probleme auf [7]. Biliäre Interventionen sind mittels konventioneller endoskopisch-retrograder Cholangiopankreatografie (ERCP) bei diesen Patienten nicht durchführbar. Mönkemüller et al. haben in ihren Publikationen die technische Machbarkeit einer erfolgreichen diagnostischen und therapeutischen BAE-gestützten ERC bei Patienten mit einer derartig postchirurgisch veränderten Oberbauchanatomie beschrieben [8], [9], die sich an ähnliche, davor bereits berichtete Untersuchungen anlehnen [10], [11]. Die Möglichkeiten der BAE-gestützten ERC erscheinen im klinischen Alltag von zunehmender Bedeutung. In dieser Arbeit erfolgt eine kritische Durchsicht der Publikationen der Arbeitsgruppe Mönkemüller et al. und Analyse der technischen Parameter bei diagnostischer und therapeutischer BAE-gestützter ERC mit abschließender kritischer Zusammenfassung und entsprechendem Kommentar aus chirurgischer Sicht.
