Zusammenfassung
Die endoskopischen Verfahren sind aus der modernen Hernienchirurgie nicht mehr wegzudenken. Durch den hohen technischen Anspruch ist die Lernkurve aber vergleichsweise lang und die publizierten Vorteile können durch technische Fehler und deren Folgen (Schmerz, Rezidiv, Komplikationen) leicht verwässert werden. Im nachfolgenden Text werden die Operationsschritte der total extraperitonealen endoskopischen Hernioplastik (TEP) ausführlich erläutert mit Hinweisen auf Alternativen und Gefahren. Von der Vorbereitung, Indikationsstellung und Lagerung, von der Trokarplatzierung über die extraperitoneale Dissektion bis zur Netzplatzierung werden die Prinzipien der TEP-Technik in Bezug auf die lokale Anatomie und möglichen Komplikationen verdeutlicht. Sowohl der Text als auch der zugehörige in der Mediathek hinterlegte Film basieren auf 20 Jahren Erfahrung der Autoren mit der TEP-Technik. Beides möge dazu beitragen, dass durch sichere TEP-Anwendung die Komplikations- und auch die Rezidivrate minimiert werden kann. Denn nur so können die Patienten von den Vorteilen der Technik profitieren.
Abstract
One can no longer think about modern hernia surgery without mentioning endoscopic techniques. But due to their high technical demands the learning curve is comparatively long. And by technical mistakes and their consequences (pain, recurrence, complications) the benefits of the endoscopic techniques can easily be turned to drawbacks. The following text explains the steps of the total extraperitoneal endoscopic hernioplasty (TEP) technique in detail pointing out alternatives and risks. From preparation, indication and positioning, from trocar placement to extraperitoneal dissection and mesh placement, the principles of TEP are elucidated in respect of local anatomy and possible complications. The text as well as the accompanying video in the Mediathek are based on the authorsʼ 20 years of experience in the TEP technique. Both of them may help in safe TEP application to minimise the complication rate as well as recurrences. Then patients can benefit from the advantages of this technique.
