Zusammenfassung

Ziel: Untersuchung des klinischen Ansprechens prämenopausaler Patientinnen mit symptomatischen Uterusmyomen nach MRgFUS-Behandlung anhand des validierten UFS-QOL (Uterine Fibroid Symptom and Quality of Life)-Fragebogens.

Material und Methoden: In diese prospektive Studie wurden 54 prämenopausale Patientinnen mit symptomatischen Uterusmyomen eingeschlossen. Diese wurden anhand des USF-QOL-Fragebogens unmittelbar vor sowie 3, 6 und 12 Monate nach MRgFUS-Therapie evaluiert.

Ergebnisse: Die Rate des technischen Erfolgs lag bei 91,5 % (95,2 % abzüglich Screeningfehler). 6/54 Patientinnen (11 %) entschieden sich im Verlauf für andere Therapieverfahren (chirurgisch n = 4, UAE [Uterine Artery Embolization] n = 2) (Therapieversager), 8/54 (15 %) schieden aufgrund einer eingetretenen Schwangerschaft von der weiteren Befragung aus, 8/54 konnten nicht nachevaluiert werden. Bei den auswertbaren Patientinnen (n = 52) zeigte sich bereits nach 3 Monaten eine deutliche Beschwerdebesserung. Die Gesamtlebensqualität stieg ausgehend von im Median 64,7 (Quartilabstand QA: 49,8 – 77,6) auf 77,6 (QA: 61,4 – 87,1) (p < 0,001), nach 6 Monaten auf 78,4 (QA: 66,4 – 89,7) (p < 0,001) und nach 12 Monaten auf 82,8 (QA: 69,8 – 92,2) (p < 0,001). Der Wert für die Symptomschwere sank von initial 46,9 (QA: 28,1 – 56,2) auf 34,4 (QA: 21,9 – 43,7) (p = 0,003), nach 6 Monaten auf 28,1 (QA: 18,7 – 38,3) (p < 0,001) und blieb konstant bei 28,1 (QA: 15,6 – 34,4) (p = 0,002) nach 12 Monaten. Die Rate an behandlungsbedürftigen Komplikationen betrug 9 % (Gesamtrate 39 %). Majorkomplikationen traten nicht auf.

Schlussfolgerung: Der MRgFUS führte im 12-Monats-Verlauf zu einer signifikanten Verbesserung der Beschwerdesymptomatik mit einem signifikanten Ansprechen nach bereits 3 Monaten. Hervorzuheben sind die fehlenden Majorkomplikationen sowie die eingetretenen, erfolgreich ausgetragenen Schwangerschaften. Die Rate der Therapieversager von 11 % war gering.

Abstract

Purpose: To investigate the clinical outcome of magnetic-resonance-guided focused ultrasound surgery (MRgFUS) treatment for symptomatic uterine fibroids in premenopausal women using the validated USF-QOL (Uterine Fibroid Symptom and Quality of Life) Questionnaire.

Materials and Methods: 54 patients with symptomatic uterine fibroids were enrolled in this prospective study. The patients completed the UFS-QOL Questionnaire prior to MRgFUS treatment as well as after 3, 6, and 12 months.

Results: The rate of technical success was 91.5 % (95.2 % after subtraction of screening errors). 6/54 patients (11 %) had other treatments (surgery, n = 4; UAE, n = 2), 8/54 (15 %) dropped out due to pregnancy, and 8/54 were lost to follow-up. The remaining group showed considerable symptom relief as early as after 3 months. The median overall quality of life score increased from 64.7 (quartile range QR: 49.8 – 77.6) before treatment to 77.6 (QR: 61.4 – 87.1) (p < 0.001), 78.4 (QR: 66.4 – 89.7) (p < 0.001), and 82.8 (QR: 69.8 – 92.2) (p < 0.001) at 3, 6, and 12 months, respectively. The corresponding median symptom severity score decreased from 46.9 (QR: 28.1 – 56.2) to 34.4 (QR: 21.9 – 43.7) at 3 months (p = 0.003) and 28.1 at 6 and 12 months (QR: 18.7 – 38.3, QR: 15.6 – 34.4) (p < 0.001, p = 0.002). The rate of complications requiring treatment was 9 %, and the rate of overall complications was 39 %. No major complications occurred.

Conclusion: Our results indicate significant alleviation of fibroid-related symptoms within 12 months of MRgFUS with improvement beginning as early as 3 months after treatment. We observed no major complications, and some women became pregnant after MRgFUS. There was a low treatment failure rate of 11 %.
