Aneurysm of the Celiac Trunk. Diagnosis with Endoscopic Ultrasound (EUS). Report of a case

A 44 year old female with history of hypertension was being evaluated for abdominal pain, predominately of the epigastrium. Physical examination was normal. A CT Scan of the abdomen without IV contrast was performed; results were interpreted as a cystic tumor of the body of the pancreas. A referral to perform an endoscopic ultrasound-guided fine needle aspiration was made for the patient.

The Abdominal Ultrasound and the EUS showed a pancreas with a normal parenchymal pattern and a cystic image located between the body and tail of the pancreas, while the Doppler color effect demonstrated evidence of blood flow at the splenic artery emergency area.

A CT angiogram showed a saccular image correspondent to a Celiac Trunk aneurysm of 2.4 cms in diameter. At the base of such aneurysm the hepatic and splenic emergency seemed normal.

Results: The initial diagnosis of a pancreatic cystic lesion was dismissed. Different imaging methods used demonstrated that it was in fact an aneurysm of the Celiac Trunk.

Two months later, endovascular occlusion of the aneurism with special stents was performed with good results and the patient was discharged without complications.
