Abstract

A definite pathogenesis for Kienböck's disease (KD) has not been identifed yet. Considering our clinical experience, mechanical factors, acute trauma or repetitive minor trauma seem to not be a primary cause, but rather factors that cause symptom aggravation of an already present KD. I feel we should look for a biological more than for a mechanical cause. Probably a vascular non-traumatic process, with a minor infarction pattern in the proximal subchondral area, is nearest to the truth. Bone remodelling results from bone resorption by osteoclasts and the formation of new bone by osteoblasts. Why osteoclast action should overpass the osteoblastic activity in the repair process is still not known. Studies of genes related to osteogenesis with polymerase chain reaction (PCR) arrays are a new option. Considering avascular necrosis as a possible reactive arthritis, PCR and viral RNA analysis could help in making the diagnosis.

Zusammenfassung

Die exakte Pathogenese der Lunatumnekrose ist bislang nicht geklärt. Ausgehend von unserer klinischen Erfahrung dürften weder mechanische Faktoren, noch akute oder chronisch repetitive Traumata als eigentliche Ursache der Erkrankung in Betracht kommen, sondern sollten eher als Faktoren angesehen werden, die die Symptome einer bereits vorhandenen Erkrankung verstärken. Wir sollten eher nach einer biologischen als nach einer mechanischen Ursache suchen. Am ehesten scheint es sich bei der Lunatumnekrose um eine nicht traumatisch bedingte vaskuläre Pathologie zu handeln, die zu einer umschriebenen Infarzierung im subchondralen, proximalen Abschnitt des Mondbeins führt. Die Remodellierung des Knochens beruht auf der Wechselwirkung von Knochenresorption durch Osteoklasten und Knochenneubildung durch Osteoblasten. Dabei ist es bislang unklar, warum in der Bilanz die Aktivität der Osteoklasten diejenige der Osteoblasten überwiegt. Vielleicht kann die PCR-Analyse der für die Osteogenese verantwortlichen Gene zur Klärung der Ätiologie der Lunatumnekrose beitragen. Auch wenn man davon ausgeht, dass es sich bei avaskulären Nekrosen möglicherweise um eine reaktive Arthritis handelt, könnten PCR-Analyse und Virus-RNA-Analyse weiterhelfen.
