Hypovolämie

Zusammenfassung

Dem hypovolämischen Schock liegt eine durch einen Volumenmangel bedingte kritische Reduktion der kardialen Vorlast zugrunde. Präklinisch existieren kaum Möglichkeiten, das Ausmaß einer Hypovolämie zu quantifizieren. Dennoch ist es von entscheidender Bedeutung, bereits am Einsatzort ein drohendes Schockgeschehen abzuwenden bzw. einen bestehenden Schock in der kompensierten Phase zu halten. Über geeignete Infusionslösungen ist in letzter Zeit v. a. im anästhesiologischen Fachbereich viel diskutiert und veröffentlicht worden. Sogenannte „balancierte Lösungen” stellen einen vielversprechenden Ansatz dar, um Probleme (Dilutionsazidose, Beeinträchtigung der Nierenfunktion) mit den klassischen kristalloiden Lösungen (NaCl, Ringer-Laktat) zu vermeiden. Ähnliches gilt für die Kolloide. Die neuesten HES-Präparate haben einen geringeren Einfluss auf Gerinnung und Nierenfunktion als ihre Vorgänger mit höherem mittleren Molekulargewicht. Auch die sog. „pädiatrischen” Lösungen, die sich v. a. durch den Zusatz von Glukose und einen reduzierten Na-Gehalt auszeichnen, sind aufgrund schwerwiegender Komplikationen (Hirnödem, Hyperglykämie) in die Diskussion geraten. In diesem Beitrag wird die präklinische Therapie der Hypovolämie und des hypovolämischen Schocks beschrieben. Dabei wird insbesondere auf die Infusionstherapie und geeignete Infusionslösungen eingegangen. Einige spezielle Situationen, in denen ein hypovolämischer Schock entstehen kann, werden dargestellt. Besonderheiten bei Kindern werden an entsprechenden Stellen explizit erwähnt.

Abstract

Hypovolaemic shock is caused by a critically reduced cardiac preload derived from volume deficiency. In the prehospital setting little can be done to quantify the severity of the hypovolaemia. However, to prevent an imminent shock, respectively keeping it compensated, is of vital importance. There is a lot of discussion regarding fluid replacement especially in the anaesthesiologic section. Balanced electrolyte solutions avoid side effects such as dilution-acidosis and disturbance of renal function caused by classical solutions (saline-solution, Ringer's lactate). The same applies to colloid solutions. Latest preparations of hydroxyethylstarch have less side effects on coagulation and renal function than those with a greater molecular weight. Pediatric solutions are likely to cause serious complications such as brain edema and hyperglycaemia. This article focuses on fluid replacement and appropriate solutions in hypovolaemic shock. Special conditions which may lead to hypovolaemic shock are discussed. Pediatric distinctions will be mentioned particularly.
