„Das eosinophile Granulom des Knochens“

Zusammenfassung

Es wird über 3 Fälle von eosinophilem Granulom des Knochens berichtet. Der älteste Patient erkrankte nacheinander mit mehreren Herden. Er war bei Beginn der Erkrankung (Schädelherd) 21 Jahre alt und kam mit einem Herd am Manubrium sterni mit 24 Jahren in unsere Beobachtung. Epikritisch konnten weitere Herde am Dornfortsatz des 6. HWK und im rechten Schenkelhals geklärt werden. Die beiden anderen Patienten hatten unilokuläre Herde in der Schädelkalotte. Es handelte sich um ein 13 jähriges Mädchen und einen 17 jährigen Jungen. Die Beobachtungen werden an Hand der Röntgenbilder und der klinischen wie histologischen Befunde besprochen. Für eine Sonderstellung der Erkrankung wird vom klinischen Standpunkt eingetreten. Summary

The author reports on three cases of eosinophilic granuloma. In the oldest patient several foci occurred in succession. He was 21 years old when the diseases started (Skull focus) and came under the author's observation at 24 with a focus at the manubrium sterni. Epicritical observation revealed further foci at the spinous process of the sixth cervical vertebra and at the collum femoris. The two other patients, a girl of 13 and a boy of 17, had unilocular foci in the skull cap. The author discusses his observations with reference to the x-ray pictures and the clinical and histological findings. From the clinical point of view he pleads for a special classification of the disease. He discusses the pathology, diagnosis, prognosis and therapy (bibliography).

Résumé

Communication de 3 cas de granulome éosinophile du squelette. Le malade le plus âgé présenta successivement plusieurs foyers. Début de la maladie à 21 ans (crâne), à 24 ans apparition d'un foyer au manubrium sternal. Ultérieurement on put encore déceler d'autres foyers au niveau de la 6è apophyse épineuse cervicale et du col du fémur droit. Les 2 autres malades présentaient un foyer unique de la calotte cranienne. Présentation de l'observation, des clichés et de l'histologie de ces 2 cas, qui concernent une fillette de 13 ans et un jeune homme de 17 ans. La clinique fait penser qu'il s'agit d'une entité morbide particulière, rappel bibliographique de l'anatomo-pathologie, et des éléments du diagnostic et du pronostic.

Resumen

Se comunica acerca de 3 casos de granuloma eosinófilo de hueso. El paciente de más edad presentó sucesivamente varios focos. Al comenzar el proceso (foco craneal) tenía 21 años, llegando a nuestra observación a la edad de 24 años con un foco a nivel del manubrio del esternón. Por último se comprobó la existencia de otros focos situados en la apófisis espinosa de la sexta vértebra cervical y del cuello del fémur derecho. Los otros dos pacientes presentaban focos uniloculares en la bóveda del cráneo. Se trataba de una niña de 13 y de un joven de 17 años. Estos casos se comentan a base de los cuadros radiológicos, clínicos y hallazgos histológicos. Se afirma que esta afección constituye un cuadro clínico autónomo. Se describe la patología, diagnóstico, pronóstico y tratamiento (bibliografía).
