Zusammenfassung
Am Beispiel von zwei malignen blutenden Dünndarmtumoren wird die Problematik röntgenologischer Diagnostik bei Intestinalblutungen diskutiert. Es wird für rechtzeitige und notwendige selektive, viszerale Angiographie plädiert, auf das Kontrastmittelpooling und die Tumorsymptome hingewiesen und ihre Bedeutung für Diagnose und Prognose der Prozesse dargestellt. Bei negativem angiographischen Befund hat sich das Enteroklysma zur weiteren Diagnostik von Dünndarmprozessen als eine sehr wertvolle Untersuchungsmethode erwiesen. Das gefäßreiche Leiomyosarkom des Dünndarms ist angiographisch, das gefäßarme Adenokarzinom des Dünndarms durch Enteroklysma zu erfassen.
Summary
Problems concerning the radiological diagnosis of bleeding, malignant, small bowel tumours are discussed with reference to two cases. The need for early selective visceral angiography is stressed; the significance of contrast pooling is pointed out and the value of recognising the tumour in arriving at a diagnosis and prognosis is discussed. In the presence of negative angiographic findings, the small bowel enema has proved a very valuable method of examination. The vascular leiomyosarcomas of the small bowel are best demonstrated angiographically, whereas the avascular adenocarcinomas are best shown by small bowel enema.
