Zusammenfassung
Nach Darstellung der Pathomorphologie und Pharmakodynamik des Karzinoids und der Karzinoidose wird über 3 eigene Beobachtungen berichtet. Die konventionelle radiologische Untersuchung des Magen-Darm-Trakts führt nur selten zum Nachweis eines Dünndarmkarzinoids und erbringt meist nur unspezifische Befunde, da die Karzinoide in der Regel submukös wachsen und oft sehr klein sind. Dagegen ist das angiographische Bild des Dünndarmkarzinoids pathognomonisch. Die charakteristischen angiographischen Zeichen mit der sternförmigen Anordnung (stellate pattern) der Vasa recta und der terminalen Arkaden sowie die Stenosen der Mesenterialarterien sind indirekte Zeichen des in die Umgebung infiltrierenden und metastasierenden Tumors und dessen Fibrosierungstendenz. Die endokrine Aktivität von Lebermetastasen kann sich angiographisch durch vermehrte zentrale Kontrastmittelanreicherung bei aufgelockerter Intermediärzone mit scharfer Randbegrenzung manifestieren.
Summary
After a discussion of the morphology and pharmacodynamics of carcinoids and of carcinoidosis, the authors describe three cases of their own. Conventional radiological investigation of the gastro-intestinal tract rarely leads to a diagnosis of small bowel carcinoids and is usually non-specific, since carcinoids are often sub-mucosal in position and frequently very small. On the other hand, the angiographic appearance of small bowel carcinoids is pathognomic. The characteristic angiographic signs consist of a stellate pattern of the terminal arcades and the vasa recta; stenoses of the mesenteric arteries are an indirect sign of infiltrating and metastasising tumours with a considerable tendency to fibrosis. Endocrine activity of liver metastases can be judged angiographically by the presence of central contrast accumulation, with a less dense intermediary zone and a sharp margin.
