Wachsende Schädelfraktur eines Neugeborenen als seltene Komplikation bei Vakuumextraktion

Zusammenfassung
Es wird über einen ungewöhnlichen Fall von wachsender Schädelfraktur eines reifen Neugeborenen mit Abhebung eines umschriebenen Kalottenstückes nach Vakuumextraktion berichtet. Der Fall weist darauf hin, daß neben meist spontan heilenden Schädelfissuren und -frakturen und neben oft tödlichen Hirnblutungen auch andere schwerwiegende therapiebedürftige Komplikationen nach der Vakuumextraktion auftreten können. Die Entstehungstheorie der sog. wachsenden Fraktur und ihre Therapie in unserem Fall werden beschrieben.
Summary
An unusual case is described of an increasing skull fracture in a mature neonate, resulting from elevation of a skull fragment during vacuum extraction. This case indicates that vacuum extraction may lead to fissures and fractures which usually heal spontaneously, as well as to fatal cerebral haemorrhage and other serious complications which require treatment. Theories regarding the development of fractures which increase in size, and of their treatment, are discussed.
Résumé
Exposition d'un cas inhabituel d'une fracture du crâne observée chez un nouveau-né, né à terme avec soulèvement d'un fragment de la calotte à la suite une extraction par le vide. Le cas démontre qu'outre les fissures et les fractures du crâne, la guérison spontanée et les hémorragies cérébrales souvent mortelles d'autres complications graves nécessitant un traitement peuvent se produire à la suite d'extraction par le vide. La théorie de la formation de ces fractures et leur traitement sont exposés dans le cas présenté.
Resumen
Se comunica sobre un caso no corriente de fractura del cráneo creciente de un recién nacido maduro con alzamiento de un pedazo de calota circunscrito después de la extracción del vacío. El caso alude a que junto a fisuras y fracturas del cráneo, que se curan espontáneamente y junto a hemorragias cerebrales a menudo mortales, pueden presentarse también otras complicaciones que provienen de la difícil terapia después de la extracción del vacío. En nuestro caso se describen la teoría del surgimiento de la llamada fractura creciente y su terapia.
