Fehldiagnosen bei traumatischen arteriellen Aneurysmen der oberen Extremität

Zusammenfassung
Ein thrombosiertes Aneurysma der A. brachialis und ein Aneurysma der A. ulnaris hatten auf Grund der klinischen Symptome jeweils nach einem Bagatelltrauma zur Fehldiagnose Fremdkorpergranulom bzw. Abszeß geführt. Histologisch handelte es sich um sog. Dehnungsaneurysmen. Die Serien-Angiographie sicherte práoperativ die Diagnose.
Summary
The author describes a thrombosed brachial artery aneurysm and an aneurysm of the ulnar artery following minor trauma which had been diagnosed clinically as a foreign body granuloma or abscess. Histologically they proved to be expanding aneurysms. The pre- operative diagnosis was made by serial angiography.
Résumé
Un anévrisme avec thrombose de l'artère brachiale et un anévrisme de l'artère cubitale furent faussement interprétés sur la base des symptômes cliniques — du moment qu'il y avait un traumatisme bénin — comme un granulome d'un corps étranger et comme un abcès. L'histologie révéla qu'il s'agissait d'un anévrisme par “pulsion”. L'angiographie en séries assure le diagnostic préopératoire.
Resumen
Un aneurisma trombótico de la A. braquial (humeral) y otro de la A. cubital habian ocasionado, por sus sintomas clinicos consecutivos a traumatismos sin importancia, diagnósticos erróneos de granuloma por cuerpo extraño y absceso, respectivamente. El examen histológico reveló que se trataba de aneurismas por dilatación. La angiografia seriada confirma el diagnóstico antes de operar.
