Die operativen Verfahren zur dauerhaften Verhütung der Ösophagusvarizenblutung sind ziemlich grobe und wenig befriedigende Methoden, die auf einem medizinisch und therapeutisch besonders ungünstigen Gebiet eingesetzt werden. Solange nichts Besseres zur Verfügung steht, müssen wir uns dieser Operationen bedienen. Sollen weitere Erfolge erzielt werden, muß der entgegengesetzte Weg beschritten werden, den die Entwicklung bisher genommen hat. Überzüchtete Indexberechnungen und operative Versorgung der ohnehin am wenigsten Gefährdeten werden nicht weiterhelfen. Die Forderung mag unpopulär sein und radikal erscheinen, aber die Zahlen sprechen eindeutig dafür: Man sollte die Shuntoperation vom Intervall in das Stadium der akuten Blutung vorverlegen.

Moreover they are employed in a particularly unfavourable field both medically and therapeutically. As long as no better methods are available these operations must be used. In order to produce more success a contrary path to that which the development has taken up to now must be followed. Statistics carried to extremes and operative care for those in the least danger will not help further. These demands will not be popular and will seem to be too radical but the figures undoubtedly support them: the timing of the shunt operation should be brought forward from the interval following a bleed to the period during the acute bleed.

Las técnicas quirúrgicas para impedir permanentemente la aparición de hemorragias por varices esofágicas son métodos poco satisfactorios y bastante crasos que se emplean en un territorio muy desfavorable desde el punto de vista terapéutico y médico. En tanto no haya algo mejor, tenemos que seguir valiéndonos de ellas. Si sehan de lograr otros éxitos, se tiene que seguir el camino opuesto a que ha tomado el desarrollo hasta ahora. Los cálculos forzados de porcentaje y los cuidados quirúrgicos de los menos expuestos no seguirán ayudando. La pretensión puede que sea impopular parezca radical pero las cifras hablan claramente a su favor: se debería adelantar la operación de shunt del intervalo al estadio de la hemorragia aguda.
