Zusammenfassung
Bei zwei Patienten im Alter von 35 und 72 Jahren sprachen die klinischen, blutchemischen und morphologischen Befunde für eine aggressive Hepatitis. Als Besonderheit konnte in beiden Fällen im Serum eine Makroglobulinämie mit einem Sedimentationskoeffizienten S20 von 16,3 bzw. 14,6 mit Fehlen des IgA und im einen Fall auch mit Verminderung des IgG festgestellt werden. Es wird angenommen, daß ein direkter Zusammenhang zwischen der Makroglobulinämie und der hepatozellulären Infiltration als Quelle dieser Makroglobuline besteht.
Summary
Clinical, biochemical and morphological results in a 35-year-old patient indicated florid acute viral hepatitis, while in a 72-year-old patient they pointed to active chronic hepatitis. In both instances there was macroglobulinaemia with a sedimentation coefficient S20 of 16.3 and 14.6 respectively, with absent IgA and, in the second case, also a decrease in IgG fraction. It is suggested that there is a direct relationship between the macroglobulinaemia and the hepatocellular infiltration, the latter being the source of the macroglobulins.

Hepatitis agresiva con gammapatía monoclonal y defecto de IgA
Los hallazgos clínicos, morfológicos y químicos de la sangre de dos pacientes de 35 y 72 años de edad hablan a favor de una hepatitis agresiva. Como particularidad pudo constatarse en el suero de ambos casos una macroglobulinemia con un coeficiente de sedimentación S20 de 16,3 y 14,6 respectivamente con ausencia de la IgA, y en un caso también con la disminución de la IgG. Se considera que existe una relación directa entre la macroglobulinemia y la infiltración hépatocelular como fuente de esta macroglobulina.
