Zusammenfassung
Die selektive Blutentnahme aus den Schilddrüsen- und Halsvenen und die radioimmunologische Parathormonbestimmung in den gewonnenen Serumproben ist eine zuverlässige Methode in der Allgemein- und Lokalisationsdiagnostik beim primären Hyperparathyreoidismus. Mit Hilfe dieser Methode konnten neun von zwölf Epithelkörperchenadenomen bei zehn Patienten gefunden werden. Bei drei Patienten mit rezidivierender Nephrolithiasis, Hypercalciurie und Normocalcämie fanden sich normale Hormonmuster in den Halsgefäßen, ebenso bei einer Patientin mit paraneoplastischer Hypercalcämie. Die Methode ist besonders bei schon voroperierten Patienten zur Lokalisation des pathologisch sezernierenden Parathyreoidea-Gewebes geeignet.
Summary
Selective blood sampling from the thyroid and neck veins and radioimmunological parathormone estimations in the samples obtained is a reliable method for general diagnosis and localization in primary hyperparathyroidism. Using this method nine out of twelve parathyroid adenomas could be found in ten patients. Three patients with recurrent nephrolithiasis, hypercalciuria and normocalcaemia had normal hormone patterns in the neck veins as did one female patient with paraneoplastic hypercalcaemia. This method is particularly suitable in previously operated patients for localization of pathologically-secreting parathyroid tissue.
