Zusammenfassung
Allopurinol hemmt die Synthese der Harnsäure im Körper und verringert dadurch den Harnsäurespiegel im Plasma und die Harnsäureausscheidung durch die Niere. Auf Grund dieser Eigenschaften ist das Mittel bei allen Krankheiten indiziert, bei denen es zu Gichtanfällen oder zur Bildung von Harnsäuresteinen kommt. Bei der Behandlung der Gicht mit Allopurinol werden die Anfälle rasch leichter und allmählich auch seltener. Von 25 Patienten waren nach einer Behandlungszeit von drei bis 18 Monaten nur zwei nicht deutlich gebessert. Bei Nephrolithiasis wurde die Hämaturie seltener oder geringer, Steinkoliken hörten auf. Nebenwirkungen, die mit Sicherheit auf das Allopurinol zurückgeführt werden könnten, wurden nicht beobachtet. In der Behandlung der Gelenkgicht ist das Allopurinol der erfolgreichen Anwendung der Urikosurika gleichwertig. Fälle, in denen die Harnsäurespiegel durch Urikosurika nicht gesenkt werden können, sprechen auf Allopurinol an. Ist die Gicht bzw. die Hyperurikämie mit Zeichen der Nierenschädigung oder der Nephrolithiasis verbunden, so halten wir Allopurinol heute für das Mittel der Wahl, da es die Harnsäureausscheidung verringert und dadurch die Tendenz zur Harnsäureausfällung im Parenchym oder im Harn aufgehoben wird. Die Auflösung bereits gebildeter Konkremente durch Behandlung mit Allopurinol ist möglich.
Summary
Allopurinol increases the synthesis of uric acid and in this way decreases the plasma uric-acid level and the renal excretion of uric acid. Because of these properties the drug is indicated in those cases in which gouty attacks or the formation of uric-acid stones occur. Allopurinol attacks progressively decreased the frequency and severity of gouty attacks. Of 25 patients treated for three to 18 months only two were not markedly improved. In the presence of nephrolithiasis haematuria became less frequent or less marked and there were no more colics. No side-effects were observed which could with certainty be ascribed to allopurinol. For gouty joints allopurinol had the same effect as uricosuric drugs. In some cases allopurinol was successful in reducing uric-acid level where uricosuric drugs had previously failed to do so. If gout or hyperuricaemia with signs of renal damage or nephrolithiasis is present, allopurinol is the drug of choice because it decreases uric-acid excretion and thus the tendency to precipitation of uric acid in the renal parenchyma or in urine. Allopurinol, however, is not capable of dissolving calculi.

El Allopurinol en el tratamiento de la gota y de la nefrolitiasis de ácido úrico
El Allupurinol inhibe la síntesis del ácido úrico en el organismo, reduciendo así la uricemia en el plasma y la eliminación del mismo por el riñon. Debido a estas propiedades está indicado en aquellas enfermedades en que aparecen ataques gotosos o formaciones de cálculos de ácido úrico. En el tratamiento de la gota con Allopurinol, los ataques gotosos llegan a ser rápidamente más leves y paulatinamente más raros. Sólo dos pacientes de un total de 25 tratados de 3 a 18 meses no habían mejorado claramente. En la nefrolitiasis llegó a ser la hematuria más rara o insignificante y los cólicos renales cesaron. Efectos secundarios, que se podrían atribuir con seguridad al AUopurinol, no se observaron. En el tratamiento de la gota articular equivale el Allopurinol al empleo satisfactorio de los uricosúricos. Los casos en que la uricemia no puede rebajarse por los uricosúricos reaccionan al Allopurinol. Si la gota o la hiperuricemia está asociada con los signos de la lesión renal o nefrolitiasis, entonces consideramos el Allopurinol, hoy día, como el medicamento de elección, ya que reduce la eliminación del ácido úrico y con ello se suprime la tendencia a la precipitación en el parénquima o en la orina. Es posible la disolución de concrementos ya formados por el tratamiento con Allopurinol.
