Zusammenfassung
In einer Untersuchung postoperativer psychopathologischer Auffälligkeiten nach Herzoperationen unter der Bedingung der extrakorporalen Zirkulation ließen sich drei Symptomkomplexe psychopathologischer Auffälligkeiten identifizieren, welche unterschiedliche psychologische Prädiktoren aufwiesen:
Als Prädiktoren einer mehr depressiven postoperativen Symptomatik können familiäre Probleme und der Mangel an Zukunftsperspektive des Patienten gelten, während Patienten mit Desorientierung und anamnestischen postoperativen Störungen präoperativ vermehrt berufliche Sorgen angeben und sich in ihrem Beruf überfordert fühlen.
Als Prädiktoren einer postoperativen paranoid-halluzinatorischen Symptomatik erweisen sich eine stark ausgeprägte präoperative Angst, das Empfinden der Wartezeit als starke nervliche Belastung, mangelndes Vertauen in die Ärzte, ein instabiles Sozialgefüge sowie der Wunsch, postoperativ nicht mehr zu arbeiten, ohne daß jedoch konkrete Zukunftspläne bestehen.
Summary
In a study of postoperative psychosis after open heart surgery three psychopathological syndromes were identified which had different psychological predictors.
Predictors of postoperative emotional disturbances are family problems and the lack of plans for the future, whereas patients with postoperative disorientations seem to have difficulties in their jobs and therefore feel distressed preoperatively. Predictors of the paranoid syndrome after the operation are: a high degree of fear in awaiting the operation and little confidence in the doctors. The social surroundings of these patients are often unstable and although they have no precise plans for the future they object to start working again after operation.
