Comparison between Off-Pump and On-Pump Beating Heart Coronary Artery Bypass Grafting

We have been performing coronary artery surgery since 2003 employing both on-pump and off-pump techniques with the overarching goal of optimizing patient outcomes and safety.

We would like to add our experience to the interesting article by Dr. Matsuhashi et al.[1]

The on-pump beating heart coronary artery bypass grafting (ONBHCABG) technique helps the surgeon to gain confidence in beating heart procedures during their off-pump learning curve period.

On the other hand, in our experience, verticalization of the heart to commit anastomoses to the circumflex artery (Cx) and its branches during ONBHCABG is limited by the right atrium cannula, leading to less revascularization.

It is worth noting that there is another option within the off-pump technique wherein the aortic and right atrium cannulas are inserted as a precautionary measure, although they may not be used.
