Management des vorzeitigen Blasensprungs

Ein vorzeitiger Blasensprung, d. h. ein Blasensprung vor Einsetzen der Wehentätigkeit (sog. PROM), manifestiert sich bei ca. 11% aller Schwangerschaften. Dabei tritt er nur in 3% der Fälle vor der 37 + 0 Schwangerschaftswoche (SSW) auf; er wird dann als früher vorzeitiger Blasensprung bezeichnet (PPROM). Das klinische Management ist entscheidend vom Zeitpunkt des Blasensprungs abhängig.

Kernaussagen
  • Der vorzeitige Blasensprung ist ein häufiges geburtshilfliches Ereignis (ca. 11%).

  • Das Vorgehen hängt maßgeblich vom Zeitpunkt des vorzeitigen Blasensprungs ab.

  • Infektionen stellen die wichtigste und häufigste Komplikation dar.

  • Bei korrektem Management ist das perinatale Outcome ab der 37. SSW kaum beeinflusst.

  • Ein Blasensprung vor der 24 + 0 SSW stellt eine Extremsituation dar. Hier muss aufgrund der besonderen mütterlichen und kindlichen Risikokonstellation individuell beraten werden.
